All food trucks must be a member of the Medallion Program within the City of Palatka. This is a onetime fee of $50 per year thru 12/31/2023. Once you are approved you are not required to go through further inspection for a 12- month period.
The Azalea festival will NOT pay or perform any Food Truck inspection at the start of the festival. I, the undersigned, have read and agree to all terms and conditions set forth in the Florida Azalea Festival Vendor Application and Rules & Vendor Information sheet (attached). I further agree to hold harmless Downtown Palatka Inc., and its officers, City of Palatka, Putnam County and its Commission, their clients, affiliated organizations or entities, directors, employees, duly appointed representatives and volunteers of any and all liability, demands, claims, and responsibility for any injury, damages or loss sustained by exhibitors, guests, or any other participants before, during or after the Azalea Festival as a result of participating in the Azalea Festival and/or as a result of displaying of my work, equipment or materials. I understand that the Festival management reserves the right to remove anything that does not meet the standards set forth in this application. I understand that there are no refunds or rain checks due to inclement weather. I agree and accept responsibility to pay taxes for any sales made during the event. In consideration of acceptance of my application, I agree to accept all decisions made by the Azalea Festival management. No refunds within 15 days of event.

Entire fee for all vendors must be received with application by 2/17. Personal or business checks must be received with application 30 days prior to festival. Otherwise a money order or Cashier’s check will be necessary. Payments are to be sent to Revitalize Historical Palatka, 110 S. 2nd St, Palatka, FL 32177.
